Printable New Patient Forms can be found below to fill out at home and bring with you, or please arrive 15 minutes prior to your scheduled appointment time to fill out the paperwork at the office.
INSURANCE
Some insurance plans cover chiropractic care. Please check with your insurance provider to determine your benefits. We are in network with several insurance carriers, and we can answer any questions regarding if we are a preferred provider for your carrier. We are also happy to submit your insurance claims electronically for you.
PAYMENT OPTIONS
For your convenience, we accept cash, check, debit, and all major credit cards. We require payment at the time services are provided, unless you have made other arrangements with us prior to your appointment.
